For questions about what UHZ services and procedures your provider covers, follow the steps below to gather the information you need.

Organize your information
Keep your insurance card handy in case you need to provide your policy and/or group number.

Call the number on your insurance card.
Ask to speak to your plan supervisor or a customer service representative. (Make note of the person’s name, the date and time.) |
| Step 3 |
Say, "Hi. I'm calling to find out if UHZ Sports Medicine Institute ( Coral Gables Specialty Physician) is in my network or if I need a referral."
"What services are covered? What are my co-payments, if any? What is my deductible? If UHZ is not in my network, what do I have to pay?" |

Billing Policies
Many insurance plans require that you make a co-payment (co-pay) at the time of you visit. You may oay be cash, check, Visa, MasterCard, Discover or American Express.
